The transition from school to adulthood for youth with disabilities and special healthcare needs can be challenging. It can be difficult to know where to begin or what the priorities should be. This FREE virtual training, which includes Charting the Lifecourse Tools, will help parents navigate this process and develop a vision and transition action plan with their youth.
This training is for parents or guardians of youth age 14 (or turning 14 soon) who have an IEP or
504 plan.
During this training, parents will…
• Gain an understanding of their changing role;
• Complete homework individualized to their youth;
• Develop a customized Vision and Action Plan.
