It is never too early or too late to start planning for life after high school – this is the training to help
prepare parents of youth age 14 for transition after high school. Got a 14-year-old? Sign up! Have a
youth ages 15-21? Sign up! This training is for you!

This training includes Charting the LifeCourse Tools to help you navigate the transition process
and is FREE!

During this training, parents will…
Gain an understanding of their changing role;
Complete homework individualized to their youth;
Develop a customized Vision and Action Plan.

The transition from school to adulthood for youth with disabilities and special health care needs can be challenging. It can be difficult to know where to begin or what the priorities should be. This training is specifically for invited parents who have children aged 14, the start of transition age.
